Remarks: We are pregnant again. After four miscarriages, the last an ectopic, we found I tested positive for two blood disorders, MTHFR and Lupus Anitcoagulant. After my last miscarriage in May, I began an 81mg aspirin regimen and increased my folic acid. My quants on Wednesday were 285 so, I’ll have an ultrasound on Monday to rule out an ectopic pregnancy. Dr. Berger was very supportive back in June when I learned of the clotting disorders. I pray this pregnancy goes the distance.

Remarks: I did the dye test and the test showed both tubes still occluded. I thought I had the flu nausea, girls at work teasing me about being pregnant. I took 3 pregnancy tests, all positive, went to hospital last night. 5 weeks 2 days HCG 1800 vag ultrasound positive -baby in right spot, but still possible ectopic? Makes no sense! Cyst noted on ovary  - I go to OB Monday for a follow up HCG test, no bleeding, slight cramping on left side. Thank you, Janet

Remarks: I’m scheduled for a check up with the Ob on Wed (11/19/08). They plan to do a vaginal ultrasound and exam and if everything looks ok I will have a regular ultrasound on Thursday (11/20/08). I feel pretty good. No real nausea, but I do feel fatigued. We are not sure when my last period was because I had a period on 9/30 (which was normal) and then I had a lite period about 14 days later that lasted about 3-4 days. It was more than spotting but less than a normal period, so I’m not sure if that was implantation bleeding or another period. The doctor is going on the 9/30 date for now which would make me about 7 weeks.

Remarks: After tubal reversal I had 2 miscarriages and 1 ectopic pregnancy. We went ahead and did the invitro and we had twins on 5/03/07. after the twins we did not think we could get pregnant on our own. I became pregnant on February 9/2007 and had positive pregnancy test 2/22/07 and I delivered on 10/18/08. his name is Zachary William Hehl. He is the first natural pregnancy that went to term since tubal reversal. I don’t know what happened to my tubes during the pregnancy with the twins, however it must have helped or it was a miracle and maybe I won’t have any more! Zachary is my 10th child.

Remarks: Positive pregnancy test on 10/14/08. First HCG was 15000 Second 25000 Third 35000. On 10/21/08 I had an ultrasound and the baby is in the uterus. I was 6 weeks and 3 days and we could see a flutter. I had a second ultrasound on 11/06/08 and the baby is doing great. I was 8 weeks and 5 days. The heart rate is 173 and the baby measures 2.01 cm. This is our first pregnancy since having tubal reversal surgery. We did not use any medications or ovulation prediction kits. We tried to relax and let nature take its course. I took a lot of time looking for a doctor who could perform a tubal reversal. There are many out there, and having surgery isn’t something I take lightly. It was very important for me to find the right tubal reversal doctor and the right staff to provide me with EXCELLENT care.

I found Dr. Berger and his staff at Chapel Hill Tubal Reversal Center to go above and beyond my expectations. I was able to get all my questions answered before and even after surgery. Dr. Berger and his staff provided me with great care, and were very helpful during the entire process. My reversal surgery went very well, and I healed very quickly. At my discharge they gave me instructions that I followed closely. I was very impressed by the follow up care that Dr. Berger and his staff provided. I am more than satisfied. They are a great team, and it because of the team work that makes Dr. Berger and his staff unique.

Dr. Berger,

I could sing your praises forever and, in fact, I have referred many people to you including perfect strangers who have stopped to remark on how precious our little D’Angelo is. I know all tubal reversal stories are different, and ours is also a most unique story. My husband and I have been together for sixteen years and had our first three kids in our mid-twenties. We always wanted more but after having to take custody of my husband’s siblings, we made the huge mistake of getting my tubes tied, a choice we thought was permanent.  I mourned the loss of the children we never had. As the kids all grew older and as we approached our mid-thirties, we began to talk about the “what-ifs” and after a short time on the internet researching PTLS, found your website.

This has been the most miraculous experience and we know that God led us directly to your care. Our first three kids are now thirteen, eleven, and nine. To be able to share this with our children is an experience beyond words. They were all in the delivery room with us, my daughters holding my legs and actively coaching, and my son being able to cut his new brother’s umbilical cord. A choir of tearful joy enveloped D’Angelo as he entered this world.

Our family is better and stronger for this, and we are all anxious to meet and be blessed with another baby come July.

Tubal Reversal Satisfaction November 26th, 2008

Honestly, anyone who would rate their satisfaction less than a 10 is insane. Everyone at Chapel Hill Tubal Reversal Center was wonderful. I can’t get care at my own doctor’s office as good as I got from the staff at your facility. I just want to thank everyone there for the care my husband and I received. We couldn’t have chosen a better place to have our tubal reversal procedure done.

A New Beginning November 24th, 2008
I had my surgery done 20 Nov, and am now sitting here at home, a little sore, but healing very nicely. I have been reflecting on my experience for the past couple of days and how it has changed us as a family already.

Doctor Monteith, I’m sure you know how you make a difference in people’s lives, given the nature of your work, and you must hear words of gratitude quite often. You gave us hope. The magic and joy of even having the possibility to conceive fills every corner of our house. We all feel it, even our daughters. Sir, you were so nice to us, and down to earth. We thank you.

Doctor Hertz, the anesthesia and getting put to sleep was the scariest part for me. You were so unbelievably kind and very patient from the beginning. We trusted you, and on the morning of the surgery, I felt no fear. We thank you as well.

To all the nurses, Martha, Donna, Fran, Lorie, and Susie thank you all so much for your kindness, and your smiles. When I posted on the message board about my experience, I told everyone that I was treated with a tenderness that is almost nonexistent in most health care institutions today. You all are very special.

Ultimately this was one of the best experiences of my life, not just because it marks a new beginning, but because it is just that type of establishment: first rate, professional, knowledgeable, affordable, organized, and caring. Even the hotel stay was perfect! And finally, Dr. Berger, I did not meet you, but this clinic is yours, and we thank God for you and that you decided to open it. You must be proud of this amazing staff.

I am very happy that I chose Chapel Hill to perform my tubal reversal surgery and would not change anything about it. My whole experience from start to finish was just great. The staff, nurses, doctors, anesthesiologist EVERYONE was just wonderful. Everyone made sure to explain everything that was happening or going to happen. I had my tubes untied by Dr. Berger on April 3rd and on May 21st I found out that I was pregnant. On November 12th, I gave birth to a 1 pound 10 ounce baby girl, Danaysia. She is doing just fine.

Remarks: I have concerns about an ectopic pregnancy and about my HCG level. My gut instinct does not leave me concerned about a miscarriage, but ectopic pregnancies… more so. Can you please provide any insight? My blood test results from yesterday, 5/27/08, showed Progesterone Assay of 22.80 and Beta HCG of 2,406. The HCG level was much higher than I would have expected given that I’m only 4 1/2 weeks out from the 1st day of my last period and only 3 weeks out from the date of conception - 5/6/08 - we didn’t even really try this month because we were a bit surprised to have had no luck in prior months, were taking it easy, and my OPK results were confusing me. I had understood that HCG levels are typically 1,500 - 2,500 in the 8-10 week range. Am I misinformed about HCG levels or indeed, do these seem high at this early stage? With HCG level already so high, I’m guessing my OB/Gyn will order an ultrasound next week, but I’m concerned that it’s so early that the sac won’t be visible. Alternatively, do HCG levels this high suggest multiples? My husband and I have no history of this with our first 3 children or in our families and we certainly didn’t do any fertility. I hope to speak to my Ob/GYN tomorrow. I got these results from her nurse today. Upon my questioning/wondering, the nurse said nothing definitive but asked me some questions about my dates this month and last month - my sense is she was wondering if I was pregnant during my previous cycle which began 4/2/08. Thanks much for any insight!

Dr. Berger Talks About Post Tubal Ligation Syndrome

PTLS Diagnosis and Tests

An email message received today about Post Tubal Ligation Syndrome (PTLS) from a patient who is scheduled for tubal reversal next month prompted this blog topic. I thought it would provide helpful information to others with similar concerns.

“My problem is horrible PTLS. I had my tubes tied in 05 at the time of my 3rd C-section. I understand that PTLS is not widely accepted or recognized in the medical community as a true syndrome or diagnosis, but could I expect to see something supporting that diagnosis in any blood work or hormone tests? Would it be normal to see abnormal hormone levels to some degree in support of this diagnosis or is it possible that all my levels would be ok but that the disruption in the flow of blood between ovaries and uterus could cause all of my symptoms and simply not show changes in hormone levels? Also, are there specific hormone tests I can have my ob/gyn check?

I only came across all this info on the net while desperately searching for answers to my growing list of symptoms that I never thought were related. This is very nerve wracking and exhausting. Three years is a long time and I look forward to this journey coming to an end and new one beginning, but I have so many questions and I was hoping you could steer me in the right direction so I can have any other diagnostics tests that may be helpful to my cause.”

Dr. Berger’s Response

One of the reasons that PTLS is not recognized as a true syndrome by most physicians is that there is no specific diagnostic test that is correlated with symptoms. If you search through the medical literature, you will find many studies with conflicting results. For example, you can do a search using Google Scholar for “hormones post tubal ligation syndrome” to review results from some recent medical studies.

Diagnostic Codes

There is no diagnostic code for Post Tubal Ligation Syndrome in the International Classification of Diseases (ICD). Without a specific diagnostic code, insurance companies do not provide reimbursement for testing or treatment. The International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) does have other diagnostic codes that can be used, depending upon the symptoms being experienced:

  • ICD-9-CM #625.0 Dyspareunia - Painful sexual intercourse.

  • ICD-9-CM #625.3 Dysmenorrhea - Painful menstruation.

  • ICD-9-CM #625.4 Premenstrual tension syndromes - Distressing physical, psychologic, or behavioral changes that occur during the luteal phase of the menstrual cycle. Symptoms of PMS are diverse (such as pain, water-retention, anxiety, cravings, and depression) and they diminish markedly 2 or 3 days after the initiation of menses.

  • ICD-9-CM #626.2 Menorrhagia - Excessive menstruation.

  • ICD-9-CM #626.6 Metrorrhagia - Irregular uterine bleeding.

  • ICD-9-CM #627.0 Premenopausal menorrhagia

  • ICD-9-CM #626.8 Disorders of menstruation, other

Insurance Coverage

Insurance companies do not recognize PTLS as a medical entity. Depending upon the symptoms you have, your doctor could use one or more of these diagnostic codes. The specific tests that may be appropriate will depend on your symptoms. For example, if you have pelvic pain, then ultrasound examination would be appropriate as part of an evaluation for pain. Abnormal menstrual bleeding would appropriately involve hormonal assays (FSH, LH, estradiol, progesterone). Based on the results of any preoperative testing, a diagnostic code could be listed on your operative report as a preoperative and postoperative diagnosis. This might influence the insurance company regarding possible reimbursement for tubal reversal surgery. Read your insurance policy carefully. If it specifically states that tubal reversal is not a covered benefit, no matter what information is sent to the insurance company, there will be no reimbursement. If the policy does not specify that tubal reversal is excluded, then there may be a possibility of getting reimbursement from your insurance carrier.

Often their local doctors tell them “It’s all in your head” and want to put them on antidepressants. Pregnant After Reverse Tubal Ligation

Mommyagain2006, a member of the Chapel Hill Tubal Reversal Center Message Board, is thrilled to announce that she is finally pregnant after her procedure to untie tubes performed by Dr. Berger in July of 2005. She achieved the pregnancy this time by undergoing IUI, which stands for intrauterine insemination, and is performed by inserting a thin catheter through the cervix and injecting sperm directly into the uterus. Mommyagain2006 is congratulated by other message board members who also want a child after tubal ligation and who are inspired by her story about pregnancy after tubes tied.

Positive Pregnancy Test After Reverse Tubal LigationI was in total shock.

Why Is It Important to take Folic Acid Before and After Tubal Reversal Surgery?

Chapel Hill Tubal Reversal Center message board members discuss the correct dose of folic acid and are advised by Dr. Berger that all women who are planning or attempting to become pregnant should take a prenatal vitamin that includes folic acid, beginning well before they start trying to conceive.

The appropriate dose is 0.4 mg to 1 mg a day. Folic acid aids in the prevention of neural tube defects, which are abnormalities in the development of the spinal cord and brain. Dr. Berger recommends that women starting taking folic acid even before trying to conceive, because it is important to have a sufficient amount stored in the body prior to conception.

For most people, 400 micrograms (0.4 milligrams)is recommended. This has been shown to decrease the risk of having a baby with a neural tube defect. Women who have had a baby with a neural tube defect may be prescribed a higher dose. Recent research also suggests that folic acid supplementation started prior to pregnancy or very early in pregnancy also reduces the risk of preterm delivery. Interestly, new research shows that folic acid and zinc supplementation for men helps boost sperm counts.

I guess I should have asked you what exactly did u mean by taking too much.I asked my Dr. the same thing & was told to just take 1/day. I did read somewhere what happens if too much is ingested, I'll post it as soon as I find it again. But I too, am taking 400mg FA. The minimum needed is 300 per day. That's why I answered the way I did. Sorry I misundertstood & if I misled you, I apologize for that too !!

Folic acid and folate (the anion form) are forms of the water-soluble Vitamin B9. These occur naturally in food and can also be taken as supplements. Folate gets its name from the Latin word folium ("leaf"). Our bodies use it to make new cells. Everyone needs folic acid. For women who are pregnant it can help prevent major birth defects of her baby’s brain and spine. These birth defects are neural tube defects or NTDs. Women need to take folic acid every day, starting before they are pregnant to help prevent NTDs.

I just wanted to let you know that I too am 33 and I go for my TR on Friday. I will be 34 in July. My first pregnancy when I was 19 was an ectopic. It was tucked away in the tube and I had to have "hip to hip" surgery to have it removed. My OB/GYN did an excellent job and saved the tube and it healed just fine. But to answer your question, having a TR only slightly increases your risk of EP. I was a very healthy 19 year old girl and still ended up with an EP. You will find that it "seems" like there is an increase in MC on here because women are on here looking for support during their hard times. Many, many go on to have full term births. Stats show that the average woman has miscarriages that go undetected because the average woman does not test until she has missed AF. After TR women test earlier to avoid a ruptured EP. Sorry so long, but I hope this helps...

I had my TR done in November of 07. I am 34. I have NEVER had any complications before TL. Never had a MC or ectopic. Since November, I have had two losses.

I have said it many times that I do not regret my TR. The chance is there to have another baby but most important to give my DH his first child. I am not sure why I had the losses. Sometimes I chalk it up to being older than 18. Other times, the early testing. Getting TR is a very personal decision and it is a very difficult journey once down this path. I do know that I have hope that my difficult journey will be rewarded eventually.

Pennylane--I just wanted to say congrats on your upcoming TR. I can't say enough wonderful things about Dr. B and his amazing staff!! The whole TR experience was incredible. I was so nervous, but everyone at CH are so great! I am 32 now, will be 33 in July. I had my TR on 11/29/07 (R 3.5, L 5.0), got my first BFP on 1/18/08, and am currently 16 weeks today, and due on 9/28/08! It is all so surreal, and such a miracle to me. The pregnancy is going great--had a few rough spots in the beginning, but all is wonderful now. Good luck to you, and rest assured that you are in the best of care! Lots of sticky babydust to you! BTW--the recovery is pretty quick, and no more PTLS symptoms for me instantly!!

Hi Penny, although I am a little bit younger I am 27 and I had my tubal reversal surgery in January and became pregnant on my 2nd cycle in March. I was nervous and scared as well, but the surgery went very well and recovery is not bad at all. You will be in the best hands possible when you are at Chapil Hill, and you will have the most support you could ever imagine through this journey of yours. I know one thing that helped me in becoming pregnant so fast is I bought the ClearBlue Easy fertility monitor. It is a bit pricey, and you can get it on Ebay for a fraction of the cost, but its well worth it.

Dr. Monteith is Joining Dr. Berger as a Tubal Reversal Surgeon

Dr. Monteith is joining Dr. Berger as a tubal reversal surgeon at Chapel Hill Tubal Reversal Center. Dr. Charles Monteith graduated Summa Cum Laude from Xavier University and received his Medical Degree from the University of California at San Francisco. Following a Howard Hughes Research Fellowship in molecular biology, he completed his residency in obstetrics and gynecology at the University of North Carolina at Chapel Hill in 2001. Since then he has been a Clinical Assistant Professor of Obstetrics and Gynecology at the University of North Carolina Medical School. Dr. Monteith is board certified in obstetrics and gynecology and has extensive experience in laparoscopic surgery and high risk obstetrics.

Explaining his decision to join Dr. Berger in practice, Dr. Monteith says, “My interest in tubal ligation reversal comes from my love of outpatient surgery and the desire to learn skills which are becoming a dying art. My hope is to be able to continue to practice tubal reversal surgery and to keep it as an option for patients who have had prior tubal ligation and desire to become pregnant.”

We never thought that we would want another child considering we have 5, 4boys 11,10,8 and 6 and 1girl 6. We were very scared when they told us twins and that is why we had my tubes tied, well that was a mistakeand at the time my Doctor didnt think it was a good idea but went along with it considering I was only 23 years old in 2001. Tell me something how likely (is it if there are twins on both sides of the family), are we likely to have twins again? Thank you for you help and another thing the most thing I am afaid of is getting surgery, in 2006 when I lost my right tube I was so scared of be put to sleep. any suggestions?

Twins do run higher in some races of people. Certain tribes from Africa have very high twin rates- can be as high as 1 in 50 pregnancies. In general, the chances of having a twin pregnancy is about 1 out of 200 pregnancies.

If you and your husband are Nigerian then I would be worried you would have a higher chance of a twin pregnancy- for the most part I have not been impressed that twin rates have been higher when patients come to me in the USA and say twins run in both families. I would say your risk of twin pregnancy would be about 1 out fo 200. If you have to take clomid about 10 out of 200 and if you have IVF about 1 out of 3!

General anesthesia is very safe when it is done in a low risk patient in a controlled situation. The only problems I have seen is in emergency surgery when everyone is rushing and the patient is high risk- say 300lbs, poor airway, heart disease, trauma, etc.

The reversal is done through a small incision- about 4 inches- just above your pubic hair line. We ask that you wait until you leave the office before you try to conceive! (the real answer is when you feel up to it- about 2-4 weeks).
