Pregnant Again with Third Tubal Reversal Baby

One of the greatest advantages of tubal reversal versus IVF is the fact that you can have unlimited pregnancies.

My husband and I, are having our 3rd TR baby.Just found out last night.
We have
1 before TR= BOY = 8yrs old
Then another BOY (TR) baby born =May 2006
Then another BOY (TR) baby born =Aug 2007
Pregnant again. Crossing fingers that it's a girl this time. Well praying it's a girl.
I'm happy, just had to share the news.

Remarks: I have concerns about an ectopic pregnancy and about my HCG level. My gut instinct does not leave me concerned about a miscarriage, but ectopic pregnancies… more so. Can you please provide any insight? My blood test results from yesterday, 5/27/08, showed Progesterone Assay of 22.80 and Beta HCG of 2,406. The HCG level was much higher than I would have expected given that I’m only 4 1/2 weeks out from the 1st day of my last period and only 3 weeks out from the date of conception - 5/6/08 - we didn’t even really try this month because we were a bit surprised to have had no luck in prior months, were taking it easy, and my OPK results were confusing me. I had understood that HCG levels are typically 1,500 - 2,500 in the 8-10 week range. Am I misinformed about HCG levels or indeed, do these seem high at this early stage? With HCG level already so high, I’m guessing my OB/Gyn will order an ultrasound next week, but I’m concerned that it’s so early that the sac won’t be visible. Alternatively, do HCG levels this high suggest multiples? My husband and I have no history of this with our first 3 children or in our families and we certainly didn’t do any fertility. I hope to speak to my Ob/GYN tomorrow. I got these results from her nurse today. Upon my questioning/wondering, the nurse said nothing definitive but asked me some questions about my dates this month and last month - my sense is she was wondering if I was pregnant during my previous cycle which began 4/2/08. Thanks much for any insight!

Remarks: I had my tr in March 2005. I did try clomid and IUD twice. Both were not successful. I have not used clomid or ovulation packets for over 6 months. Just left it in God’s hands. I am 43 years of age and I am always 26 days on my cycle. I did have it confirmed that one of my tubes is blocked from scar tissue so I only have my left one open. I missed my cycle and have not been feeling very well for the last week or so. I bought a pregnancy test and it was positive. I bought 4 more and they are all positive. I did this yesterday late afternoon. By the time I took the test the Dr’s office was closed so I did not get a chance to call but I am planning on calling Monday morning.

Remarks: I’m not entirely sure how this happened!!! LOL - I must have ovulated right after my period stopped??? I don’t really ‘feel’ pregnant yet, the only reason I tested was because I thought I started bleeding/spotting again 2 days after my cycle stopped. Then it stopped after 2 days and started again 2 days later. I’ve never ever had spotting or any kind of break thru bleeding-except when I was pregnant. Also, I was having pain on my right side that was strong enough to wake me from sleep Friday night. We did the hpt and that line came of fast. We aren’t talking a faint line, like I would have expected on CD16!!!(if any line at all) My Dr. sent us to the Hospital since I was having pain on Rt. side. The pain went away and my hcg level was 549. Although it was too early to see anything, they did an ultrasound anyway. Nothing. I test today for 2nd level. I still have some spotting, not like I did on Saturday. Just playing the waiting game and praying for our miracle. Funny how life changes in a moment! Thanks!!

Remarks: I have had 2 miscarriages and one ectopic (lost my right tube). We weren’t trying because my HSG showed scar tissue even though I did see with my own eyes the dye go all the way through. We weren’t TTC since I was told I didn’t ovulate without Clomid (I have never test positive for ovulation with test sticks or a fertility monitor without being one Clomid). We are in total shock and of coarse overjoyed and scared at the same time. My 1st HCG number at CD 31 was 21 at CD 33 39 (that was 45 hours apart). I will keep you updated.

Remarks: I used ovulation test sticks (7 stick packs), monitored daily temperature and monitored my cycle. In March (period began 3/1) the peak ovulation day was never indicated. In April I had what appeared to be a very brief (2 1/2 days) period beginning 4/1. Noticed swollen and tender breast on 4/10. Began my usual ovulation stick test on 4/11. On the morning of 4/13 I took both an ovulation test and a pregnancy test and the pregnancy test came up positive. Noticed some spotting that day. Had a quantitative analysis done at ob/gyn on 4/14 which confirmed the pregnancy. Monday 4/14 Progesterone 3.87 and HCG 144.1. Due to the low numbers I was put on Progesterone suppositories and pelvic rest. Wednesday 4/16 Progesterone 22.25 and HCG 176.8. Today, Friday 4/18 HCG 299.00. I continue daily spotting and remain on Progesterone suppositories. I am scheduled for another quantitative analysis on Monday 4/21.

Remarks: Although my husband and I were extremely excited to find the pregnancy test positive on Friday, April 4th, I did start having some mild pains on my right side. The next day I had no pain at all, but Sunday morning I wasn’t feeling so well. I got up and went to church but I started to have some intense pain. I went directly to the ER. They did an ultrasound and found a mass in my right tube. The doctor could not verify that it was a sac and decided to do some exploratory surgery to find out exactly what it was. It was in fact an ectopic pregnancy in the tube which had to be terminated. Thankfully, there was no damage to the tube or any surrounding tissue. We felt that God had already given us a name for this child, Moriah Grace, and we will be holding a memorial service for her this next Sunday. Thank you Dr. Berger and staff. This is not the way we wanted things to turn out, but we now know that I can in fact get pregnant. We do so appreciate the gift you have given in restoring my body. Blessings, Tim and Joyce J.

Remarks: Had M/C on 2/21/2008 one week after a + HPT. first number 59, second (a week later) 6. started feeling funny so tested on 3/15 and got a bfp. 3/19 hcg 141 3/21 hcg 318 3/26 hcg 2549 u/s on 3/31 showed gest sac, fetal pole and h/b of 97 they say I am measuring 6 weeks but at the u/s i had a m/c exactly 5 weeks 4 days before. Also found something odd on my ovary that doesn’t look like a cyst, going for another u/s in week to check it and baby again, they seem to think the heart is too slow for 6 weeks along even though I explained that I could be no further than 5 w 4 d. What do you think, is that a slow heart rate?

Dr. Berger Talks About Post Tubal Ligation Syndrome

Dr. Berger gives helpful advice to women with symptoms of Post Tubal Ligation Syndrome that may help with them with insurance coverage for testing and treatment.

Post Tubal Ligation Syndrome (PTLS)

PTLS Diagnosis and Tests

An email message received today about Post Tubal Ligation Syndrome (PTLS) from a patient who is scheduled for tubal reversal next month prompted this blog topic. I thought it would provide helpful information to others with similar concerns.

“My problem is horrible PTLS. I had my tubes tied in 05 at the time of my 3rd C-section. I understand that PTLS is not widely accepted or recognized in the medical community as a true syndrome or diagnosis, but could I expect to see something supporting that diagnosis in any blood work or hormone tests? Would it be normal to see abnormal hormone levels to some degree in support of this diagnosis or is it possible that all my levels would be ok but that the disruption in the flow of blood between ovaries and uterus could cause all of my symptoms and simply not show changes in hormone levels? Also, are there specific hormone tests I can have my ob/gyn check?

I only came across all this info on the net while desperately searching for answers to my growing list of symptoms that I never thought were related. This is very nerve wracking and exhausting. Three years is a long time and I look forward to this journey coming to an end and new one beginning, but I have so many questions and I was hoping you could steer me in the right direction so I can have any other diagnostics tests that may be helpful to my cause.”

Dr. Berger’s Response

One of the reasons that PTLS is not recognized as a true syndrome by most physicians is that there is no specific diagnostic test that is correlated with symptoms. If you search through the medical literature, you will find many studies with conflicting results. For example, you can do a search using Google Scholar for “hormones post tubal ligation syndrome” to review results from some recent medical studies.

Diagnostic Codes

There is no diagnostic code for Post Tubal Ligation Syndrome in the International Classification of Diseases (ICD). Without a specific diagnostic code, insurance companies do not provide reimbursement for testing or treatment. The International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) does have other diagnostic codes that can be used, depending upon the symptoms being experienced:

  • ICD-9-CM #625.0 Dyspareunia - Painful sexual intercourse.

  • ICD-9-CM #625.3 Dysmenorrhea - Painful menstruation.

  • ICD-9-CM #625.4 Premenstrual tension syndromes - Distressing physical, psychologic, or behavioral changes that occur during the luteal phase of the menstrual cycle. Symptoms of PMS are diverse (such as pain, water-retention, anxiety, cravings, and depression) and they diminish markedly 2 or 3 days after the initiation of menses.

  • ICD-9-CM #626.2 Menorrhagia - Excessive menstruation.

  • ICD-9-CM #626.6 Metrorrhagia - Irregular uterine bleeding.

  • ICD-9-CM #627.0 Premenopausal menorrhagia

  • ICD-9-CM #626.8 Disorders of menstruation, other

Insurance Coverage

Insurance companies do not recognize PTLS as a medical entity. Depending upon the symptoms you have, your doctor could use one or more of these diagnostic codes. The specific tests that may be appropriate will depend on your symptoms. For example, if you have pelvic pain, then ultrasound examination would be appropriate as part of an evaluation for pain. Abnormal menstrual bleeding would appropriately involve hormonal assays (FSH, LH, estradiol, progesterone). Based on the results of any preoperative testing, a diagnostic code could be listed on your operative report as a preoperative and postoperative diagnosis. This might influence the insurance company regarding possible reimbursement for tubal reversal surgery. Read your insurance policy carefully. If it specifically states that tubal reversal is not a covered benefit, no matter what information is sent to the insurance company, there will be no reimbursement. If the policy does not specify that tubal reversal is excluded, then there may be a possibility of getting reimbursement from your insurance carrier.

Dr. Berger began studying PTLS when it became clear that a certain percentage of our patients were mentioning similar symptoms. Many of the women contacted him after their surgeries to say the TR had helped them dramatically -even if pregnancy was their primary goal.

Since joining the staff of Chapel Hill Tubal Reversal Center, however, it has become apparent to me that certain women do have worsening menstral periods and other symptoms after tubal ligation. Many of these patient state they have marked improvement after reversal surgery.

Often their local doctors tell them “It’s all in your head” and want to put them on antidepressants. This is really unfortunate, since most of these women are sure all of their symptoms started after their tubal ligation. Luckily, Dr. Berger has been able to help many women relieve their PTLS symptoms by performing the tubal reversal surgery for them. Patients often report total relief of symptoms within a couple months after their reversal with Dr. Berger.

Pregnant After Reverse Tubal Ligation

Mommyagain2006, a member of the Chapel Hill Tubal Reversal Center Message Board, is thrilled to announce that she is finally pregnant after her procedure to untie tubes performed by Dr. Berger in July of 2005. She achieved the pregnancy this time by undergoing IUI, which stands for intrauterine insemination, and is performed by inserting a thin catheter through the cervix and injecting sperm directly into the uterus. Mommyagain2006 is congratulated by other message board members who also want a child after tubal ligation and who are inspired by her story about pregnancy after tubes tied.

Why Is It Important to take Folic Acid Before and After Tubal Reversal Surgery?

Chapel Hill Tubal Reversal Center message board members discuss the correct dose of folic acid and are advised by Dr. Berger that all women who are planning or attempting to become pregnant should take a prenatal vitamin that includes folic acid, beginning well before they start trying to conceive.

The appropriate dose is 0.4 mg to 1 mg a day. Folic acid aids in the prevention of neural tube defects, which are abnormalities in the development of the spinal cord and brain. Dr. Berger recommends that women starting taking folic acid even before trying to conceive, because it is important to have a sufficient amount stored in the body prior to conception.

Can you ever take too much Folic acid? My pre-nat.has 400 mcg and then one extra at 1mg?

For most people, 400 micrograms (0.4 milligrams)is recommended. This has been shown to decrease the risk of having a baby with a neural tube defect. Women who have had a baby with a neural tube defect may be prescribed a higher dose. Recent research also suggests that folic acid supplementation started prior to pregnancy or very early in pregnancy also reduces the risk of preterm delivery. Interestly, new research shows that folic acid and zinc supplementation for men helps boost sperm counts.

I guess I should have asked you what exactly did u mean by taking too much.I asked my Dr. the same thing & was told to just take 1/day. I did read somewhere what happens if too much is ingested, I'll post it as soon as I find it again. But I too, am taking 400mg FA. The minimum needed is 300 per day. That's why I answered the way I did. Sorry I misundertstood & if I misled you, I apologize for that too !!

Folic acid and folate (the anion form) are forms of the water-soluble Vitamin B9. These occur naturally in food and can also be taken as supplements. Folate gets its name from the Latin word folium ("leaf"). Our bodies use it to make new cells. Everyone needs folic acid. For women who are pregnant it can help prevent major birth defects of her baby’s brain and spine. These birth defects are neural tube defects or NTDs. Women need to take folic acid every day, starting before they are pregnant to help prevent NTDs.

Leafy vegetables such as spinach, turnip greens, lettuces, dried beans and peas, fortified cereal products, sunflower seeds and certain other fruits and vegetables are rich sources of folate. Some breakfast cereals are fortified with for folic acid. Folate is also found in Vegemite, with an average serving (5gm) containing 100μg. The established tolerable upper intake level (UL) for folate is 1 mg for adult men and women, and a UL of 800 µg for pregnant and lactating (breast-feeding) women less than 18 years of age.

The U.S. Public Health Service urges every woman who could become pregnant to take 400 micrograms (400 mcg) of synthetic folic acid every day and whilst it is unlikely that women will be hurt from taking too much folic acid supplements, unless their doctor advises them to take more, most women should limit the amount they take to 1,000 mcg a day.
